Intracavernous Injection Therapy
Intracavernous injection therapy involves self-injection of a vasoactive agent directly into the corpus cavernosum of the penis, producing an erection within 10–15 minutes regardless of sexual stimulation.
Medications Used
Alprostadil (prostaglandin E1) is the most commonly used agent and the only one approved as a monotherapy. Combination formulations (papaverine, phentolamine ± alprostadil — the "Bimix" and "Trimix") are used when higher efficacy is needed at lower doses, reducing side effects.
Who Is It For?
Injection therapy is indicated for men who do not respond to or cannot use oral PDE5 inhibitors, and who are not yet ready for or do not want surgical implantation. It is highly effective even in severe ED cases, including post-prostatectomy and neurogenic ED.
Self-Administration
Patients are trained to self-inject in the clinic. Most find the procedure simple and comfortable after instruction. The needle is very fine; discomfort is typically minimal.
Safety
The main risks are priapism (prolonged erection) and penile fibrosis with repeated use. These are minimised by proper dosing and limiting injection frequency to 2–3 times per week maximum.
